Overview

Obvious "nose jobs" are a thing of the past. Rhinoplasty has become so sophisticated that it literally leaves no telltale signs. The more natural the nose looks-the better. Subtle refinement-not drastic alteration is key in most cases. This requires that the surgeon posses artistic vision and sound aesthetic judgement- Dr. Palmer-recently voted "one of the World’s Best Facelift Surgeons"-has these talents!

According to Dr. Palmer’s revolutionary trademarked principles of beauty and aging, the nose carries little inherent facial aesthetic importance. It is to remain unobserved allowing the aesthetically more important beauty features like the cheeks, eyes and lips to be fully appreciated. Rhinoplasty can be performed to alter nasal size or shape whether it is traumatic or genetically acquired. Unattractive nasal features such as a wide bulbous tip, wide nasal bridge, large dorsal hump, malformed/asymmetric tip, crooked nose, or a deviated septum causing breathing problems are readily correctable. Even in the case of prior surgery, the nose can most often be substantially softened and the overall shape vastly improved.

Procedure

The surgery entails reshaping the nasal skeleton that is comprised of cartilage and bone. Once the nasal skeletal structure is visualized, appropriate modifications can be performed to achieve the desired results. It is frequently necessary to modify all three areas of the nose in order to assure aesthetic harmony and balance. Cartilage of the nasal tip is deftly and precisely sculpted while the nasal bones are shaped and thinned. Once this is achieved-a natural and refined appearance will follow.

The incisions are well camouflaged within the nose and there is little, pain, swelling or discomfort following the procedure. Rhinoplasty is performed here at The Beverly Hills International Center for Aesthetic Surgery’s fully accredited surgery center. Following the procedure, you will have follow up appointments with Dr. Palmer, in order to monitor your healing and progress. Most patients return to their normal routine in 1 week but avoid strenuous activity for 2 weeks and physical (rough) contact with the nose for 1 month. In the above Photo the profile reveals a hump in the bony portion of the nose.  This may give a false impression that the chin is weak and throw the profile out of balance.  The Pictures show how a Rhinoplasty Procedure was used to decrease the hump on the nose allowing it to be smaller and more balanced.  Whether this is performed using an Open Rhinoplasty or a Closed procedure, it’s very important that not too much is removed or the Rhinoplasty will leave a ski- slope profile that is a tell tale sign that Nasal Surgery has been performed.

When the nasal tip is wide or bulbous, it’s important to determine what is causing the problem before the Rhinoplasty is to be performed.  During the Rhinoplasty, there is little time for hesitation or changing the operative plan.  During Rhinoplasty Surgery, there is about a 2 hour period that bleeding may be controlled.  This is done by injecting a Novacaine like liquid within the tissues of the nose.  The liquid constricts the blood vessels in the tissue, so the surgeon can operate and adjust the tissues during the Rhinoplasty Procedure.  This is basically, the same for a Closed or an Open Rhinoplasty, although an Open Rhinoplasty does allow better visualization of the tissues and does allow more access for cauterization of bleeding vessels during the Rhinoplasty Surgery.  Dr. Francis Palmer, Co-Director of Facial Plastic Surgery at a major medical school, teaches Rhinoplasty Surgery Procedures to Surgeons in training.  Dr. Palmer believes in a complete and thorough operative plan before the Rhinoplasty begins that has been discussed with the patient.  Doing Rhinoplasty in a timely manner will most likely result in less swelling and bruising.

		Before Open Rhinoplasty Surgery, this male has a bulbous nasal tip with wide nasal bones and supratip cartilages.  The skin is thick and contributes to the thickened tip appearance.   Dr. Palmer performed an Open Rhinoplasty Procedure where the nasal tip cartilages were re-shaped for a more natural appearance.  Dr. Francis Palmer will use cartilage from behind the ear that has been shaped into a perfect tip as a further means of refining the nasal tip during Rhinoplasty.  Dr. Palmer prefers to do an Open Rhinoplasty over a Closed Rhinoplasty when tip grafts are required as the Open Rhinoplasty, Dr. Palmer believes, allows for precise suturing of the graft in place.  Dr. Palmer prefers ear cartilage over all other tissues and man-made grafts for the tip area during Rhinoplasty Surgery.

This is a before Rhinoplasty Surgery Procedure photo of a young woman that has a bit of a droopy nasal tip.  You can see that the tip appears too long and is at approximately 105 degrees of rotation.  The ideal tip angle for this woman is between 110 and 115 degrees.  The nasal bridge has a bump that is evident on profile.  Dr. Francis Palmer planned a Closed Rhinoplasty Procedure to remove the bump and rotate and refine the nasal tip into a more aesthetic shape and position using a Retrograde Tip-Plasty Technique.  This allows for maximum tip refinement with minimum swelling and recovery.

	In these Before and After Rhinoplasty Pictures, the nasal tip is rotated downward obscuring the nostril from being seen and making the nose appear too long.  After the Closed Rhinoplasty Procedure the nasal tip has been rotated into a more normal aesthetic position revealing the proper amount of nostril.  The nose no longer appears to be too long but looks balanced with the other facial features.

Pre Operative picture before the Open Rhinoplasty Surgery.  There is evidence of a prominent nasal bump on the nasal bridge consisting of both bone and cartilage.  The patient wanted to make the bump much smaller so that his profile would be more balanced.  In this before Rhinoplasty photo, the nose appears too large, on profile.

Here’s the same patient showing the Postoperative Closed Rhinoplasty Surgery (after) picture on profile.  The profile of the nose looks natural with much less of a bump.  The nose looks masculine yet balanced.  The bones were broken in this case as is typically done when a large dorsal hump is present.  A wafered plastic cast is worn for 1 week following the Closed Rhinoplasty Procedure.  Dr. Francis Palmer does not use nasal packing for his Rhinoplasties unless a Septoplasty is to be performed during the Rhinoplasty Procedure.  In that case, small pieces of telfa pads are placed in each nostril over night.  After 1 month the nasal bones are set and regular activity may be resumed.

Rhinoplasty Procedure Facts:

• Rhinoplasty is a procedure used to alter the shape of the nose.  This may include the nasal tip alone or include the entire shape of the nose.
  
  
• Rhinoplasty Procedures must take into account numerous angles and proportions…like the angle between the nose and the forehead is 145 degrees while the ideal angle between the tip and the lip is between 105 and 115 degrees (called tip rotation).
  
  
• Rhinoplasty Procedures that change one aspect of the nose may affect 2 or even 3 other areas of the nose.  For example, if the nasal tip is made smaller by removing excess cartilage the tip will be smaller.  If support has also been reduced during the Rhinoplasty, the tip may rotate downward or droop.  This drooping may cause the cartilages and bone of the nasal bridge to appear to large compared to the nasal tip.
  
  
• Rhinoplasty Procedures are typically performed as outpatient day surgery.
  
  
• Rhinoplasty Procedures take about 1-2 hours for a Closed Rhinoplasty and 2-3 hours for an Open Rhinoplasty.  The length of time is determined by how many areas, of the nose, need to be re-shaped and whether it is a first (Primary Rhinoplasty) or a Revision Rhinoplasty.  This will also determine your surgical fee.  Higher fees are charged for more challenging Rhinoplasty Procedures.
  
  
• The tissues of the nose may become quite hard and almost brittle from repeated Rhinoplasty Procedures, so keep realistic expectations and choose a highly skilled surgeon from the beginning.
  
  
• Rhinoplasty Procedures typically have minimal discomfort and swelling but this will vary with patient and surgeon.  Dr. Palmer has recommendations for medications and over the counter remedies to avoid in order to get you ready for your Rhinoplasty Surgery.
  
  
• Patients are allowed to travel back home, all over the world, after 7 days.  The cast, if applied, is a small plastic cast that is removed at 1 week post Rhinoplasty.  Dr. Palmer does not use nasal packing for Rhinoplasties.  If you are having a septoplasty for breathing correction, Dr. Palmer will use a small piece of telfa that is removed the following day.
  
  
• If the nasal bones have been surgically broken during the Rhinoplasty, Dr. Palmer advises that you avoid physical contact to your nose until the nasal bones are set.  This typically occurs at 1 month post Rhinoplasty.  Physical activity may typically be resumed after that first month post-op.
  
  
• It is advised that you do not blow your nose for three months and do not wear glasses, after your Rhinoplasty, for 4-6 months.
  
  
• Rhinoplasty Procedures are vastly different from patient to patient as is swelling and post-surgical healing.  Dr. Palmer advises his Rhinoplasty patients to use ice therapy and shows them how to massage the swelling out of their nose after surgery.  Dr. Palmer believes that this improves the end Rhinoplasty result.
