Beverly Hills Rhinoplasty by Beverly Hills Plastic and Cosmetic Surgeon, Dr. Francis R Palmer, III.

 Teaching a Revision Rhinoplasty today to other Plastic and Cosmetic Surgeons, in the OR and it came to me that Rhinoplasty and Revision Rhinoplasty takes thousands and thousands of cases performing Rhinoplasty for the Plastic and Cosmetic Surgeon to get a real handle on this difficult procedure.  Why?

 1. The nose is unlike any other body feature that we do Plastic and Cosmetic Surgery…the tissues do not react well to repeated Rhinoplasties and the tissue can be down right unpredictable during the healing phase.  It takes thousands of cases for the Plastic and Cosmetic Surgeon to get a handle on the diversity of healing that may occur and to learn ways to deal with this issue effectively.

 2. Rhinoplasty is literally a Plastic and Cosmetic Surgery of mm’s….a little off here and there is the difference between a great nose and one that’s mediocre.

 3. The other side of this is …more is better, which means that if a little off looks good, more off the top, off the bones, off the nasal tip is better and this my friends is DEAD WRONG! There is an old saying in surgery “the opposite of good is better”...this means that if it looks good, or you have a good result, trying to get a better result may wind up a disaster.  Many Revision Rhinoplasties that I perform are for this very reason…some other Plastic and Cosmetic Surgeon went too far…took off too much bone, too much cartilage, rotated the tip too much.  Whatever….but it was too far and required a repeat Rhinoplasty and a trip from all over the world to see me in Beverly Hills.

 The take home message that came to me this morning as I am directing 2 other Plastic and Cosmetic Surgeons on this Revision Rhinoplasty here in Beverly Hills is the following: “after almost 20 years of performing Rhinoplasty and Revision Rhinoplasty here in Beverly Hills…it is the added wisdom and with this comes the experience of knowing when enough is enough during the Rhinoplasty.  Said another way… it’s knowing when to be bold and when to be conservative and having the courage and conviction of knowing you’ve made the right decision for the patient….that’s what you get from years and years of performing Rhinoplasty.”  Easilly the most technically and aesthetically challenging Plastic and Cosmetic Surgery Procedure done here in Beverly Hills or anywhere around the world.

 Just got word that a Major Magazine in the UK is running a story of our “Celebrity Lift” as one of the latest innovations in Plastic and Cosmetic Surgery and the beauty biz.  I couldn’t agree more.  here are the advantages of the “Celebrity Lift” over Face Lifts and Necklifts.

 Revision Rhinoplasty cases frequently require the use of ear cartilage to rebuild the nasal tip.  Be sure to preserve this valueable asset when having Otoplasty (ears pinned).  The ear cartilage is buried under the scalp behind the ear where it can be harvested if needed for a Revision Rhinoplasty.  In fact, my staff is trained to ask “do you have any ear cartilage left” of all Revision Rhinoplasty inquiries by telephone or internet.

 I ran across this article, from Career Builders that talks about taller people making more money.  We all know that attractive people and yes, tall people seem to get more out of life…more popular, more attention from the opposite sex and more career opportunities.  Society places a premium on looks and height.  Not fair…but hey, who ever said life was fair?  These people have higher Numbers and as such do better in life…..

    I’ve had numerous Rhinoplasty and Revision Rhinoplasty consultations, this week, where the patiens seemed confused about when an Open Rhinioplasty should be done versus a Closed Rhinoplasty…so I though this would be a good time for a brief review of the subject.

   In my opinion, you should avoid a Rhinoplasty surgeon who makes claims like “I only do Open Rhinoplasty…or I only do Closed Rhinoplasty.”  Why?….because both are viable and important techniques of Rhinoplasty and the surgeon should be able to do both…but select which one is the right technique in your particular case.

  When I’m teaching Rhinoplasty to surgical residents I tell them to keep it simple.  Open Rhinoplastry is indicated, in my opinion, when there is a very droopy tip as this technique causes a minimum of 5-7 degrees of tip rotation everything else being equal.  An Open Rhinoplasty therefore would not be appropriate if the nasal tip was already at the ideal rotation or over-rotated…and if performed would require an additional procedure to place an ear grfat to the nasal tip to rotate the tip back down to the aesthetically correct position.

 An Open Rhinoplasty should always be considered when the nasal tip is asymmetric, previousy operated on (previous Rhinoplasty) or otherwise requires the placement of a cartilage graft to the nasal tip.  Why?…because an Open Rhinoplasty allows visualization of the tip cartilages as they exist in the nose, allows direct manipulation of these cartilages and allows securing/suturing of the cartilages and/or cartilage grafts under direct visualization….a Closed Rhinoplasty does not!

  A Closed Rhinoplasty is indicated for all other issues with the nose…large hump, placement of a dorsal graft, breaking the bones etc can all be done without an Open Rhinoplasty.

 The small ioncision along the front of the nose is never a reason, for me, to do or not do an Open Rhinoplasty.

 You’re in your 30′s or 40′s and you begin to notice that your face just doen’t look as good as it used to but you’re not sure what happened or what you should do to correct it.  Chances are your cheeks are loosing some of the fullness that they used to have.

 “After a tough week of many Plastic and Cosmetic Surgeries….Celebrity Lifts, Restylane/Juvederm Injections, Radiesse Injections, Botox Injections, Rhinoplasty and Revision Rhinoplasty….I’m more than ready for a few days off for R and R.  I am working on scheduling rehersals for my daughter with the new band TKO as well as some new recording studio time for new songs.  Also had some meetings with production companies for TV shows….and I have 8 chapters written for my new book.  Man, I’m beat….but what a blast!

     As I completed my last Plastic Surgery procedures for the week, which happened to be 2 Revision Rhinoplasties…one on a young man from canada and a beautiful model from an Asian country…it struck me that Beverly Hills Plastic and Cosmetic Surgery is completely global in nature. 

    This week, I’ve done Celebrity Lifts and Radiesses on patients from Singapore, China and the midwestern United States…along with Revision Rhinoplasty for other out of country patients.  The world is truly flat and information and services are readilly available for those willing to seek them out.
