When having your photo taken avoid smiling, avoid makeup and try to have your face (or area of interest) fill the entire picture frame. On profile shots be to keep the head at a 90 degree angle to the neck.

Please be sure to include some notes regarding your concerns and be sure to list any prior surgeries that have been performed in this area.
